scellone is an unstoppable thug who one day suffers a heart attack. doctor flaminio, a dentist at the mala, convinces him to have a compliant surgeon operate him in his equipped operating room. but to replace the heart of the underworld there is a need for a donor: the problem seems to be solved when scellone stumbles into the corpse of gigino, a tramp who in reality has taken drugs with a substance that caused his apparent death.
